These brief questionnaires are designed to help identify symptoms commonly associated with mental health conditions. They are not diagnostic tools. A positive screen may indicate that a comprehensive psychiatric evaluation could be beneficial.

ADHD

Adult ADHD Self-Screening Tool

Screens for symptoms of inattention, hyperactivity, and impulsivity in adults.

Anxiety

GAD-7 Anxiety Screening Tool

Screens for symptoms of generalized anxiety, including excessive worry, restlessness, irritability, and physical symptoms of anxiety in adults.

Depression

PHQ-9 Depression Screening Tool

Screens for symptoms of depression, including low mood, loss of interest, fatigue, sleep changes, and thoughts of self-harm in adults.

Mood Disorders

MDQ Mood Disorder Screening Tool

Screens for symptoms associated with bipolar spectrum and other mood disorders, including mood elevation, irritability, impulsivity, and changes in energy or behavior in adults.
