Pricing
Lee Psychiatry is a private-pay practice and does not accept or bill insurance.
Payment is due at the time of service. Upon request, a superbill can be provided for patients who wish to seek possible reimbursement from their insurance company for out-of-network services. Reimbursement is not guaranteed and is determined by your individual insurance plan.

Initial Evaluation (60 mins)
An initial evaluation is mandatory for all new patients. This appointment allows me to understand your symptoms, history, and goals so we can create an effective treatment plan.
Follow up appointment (30 mins)
Follow-up appointments are required for all active patients. Most clients are seen every 2–4 weeks in the beginning, then every 90 days depending on individual needs and medication requirements.
Self Pay Fees
For clients choosing to pay out of pocket, the following self-pay rates apply:

Paperwork Fee
As a PMHNP, I can complete a variety of mental-health forms, including work and school notes, treatment verification letters, ESA letters (housing only), medication documentation, and FMLA or short-term disability paperwork when appropriate. A $50 paperwork fee applies to all non-clinical forms. Some legal or forensic evaluations may require a psychologist or psychiatrist. Please contact the clinic to confirm whether I can complete your specific form.

No Show Fees
A 24-hour notice is required for all cancellations. Missed appointments without notice will incur a $75 no-show fee. After two no-shows, you may be dismissed from the practice.
Private-Pay Practice
Lee Psychiatry is a private-pay psychiatric practice. We do not accept or bill insurance directly.
Choosing private pay allows your care to remain focused on your individual needs without insurance restrictions or requirements.
Payment is due at the time of service. Upon request, a superbill may be provided for you to submit to your insurance company for possible out-of-network reimbursement.
