Paperwork Request Policy
To ensure timely and accurate completion of forms, the following policy applies to all paperwork requests:
Types of Paperwork Completed
I am able to complete a variety of mental-health–related forms, including:

Work and school excuse notes

Return-to-work or accommodation letters
-
ESA letters (housing only)

Medication documentation or verification
-
Treatment verification letters
-
FMLA and short-term disability forms (case-by-case)
Certain legal, forensic, or specialized evaluations may require a psychologist or psychiatrist and cannot be completed by this clinic.
Paperwork Fee
All non-clinical paperwork requests require a $50 fee.
Payment must be made in full before any paperwork is completed, processed, or released to the patient.
This fee covers administrative time, review of your chart, documentation, and provider processing time.
Payment can be made via Zelle or Venmo. Instructions will be provided once your request is received.
Processing Time

Standard processing time: 2–3 business days

Please submit forms early to avoid delays.
Requirements for Completion
To complete any paperwork:

You must be an established patient with a recent visit.

The content of the form must be clinically appropriate and supported by your treatment history.

You may be required to schedule an appointment if the form needs updated information or further evaluation.

Forms That Cannot Be Completed
This clinic does not complete:

Legal competency evaluations
-
Forensic evaluations
-
Court-ordered assessments
-
Social Security disability determination paperwork
-
Airline ESA letters
-
Custody or parental fitness evaluations
These require specialized assessments outside the scope of practice.
